 Putting Patients at the Heart Program

Trillium Health Partners, in partnership with Saint Elizabeth Health Care is embarking on a new care initiative for our patients having cardiac surgery. The Putting Patients at the Heart, or PPATH Program, offers support to patients and their families to help them transition more smoothly from hospital to home. One of the goals of this initiative is to offer patients the right care in the right place at the right time.

Key features of the PPATH Program are:

  • Access to a health care provider by telephone for any questions/concerns. This service is available 24 hours a day, 7 days a week for a period of 30 days after the patient is discharged.
  • An integrated care coordinator (ICC) who will work with patients, their significant others or families to develop a care plan that meets the patient’s needs and organizes services that they may need at home during recovery. When appropriate, some of the services may be offered remotely using a tele-monitoring platform.
  • Better communication between patients, families and the health care team, resulting in better coordinated care.
  • A shared patient care record between the hospital and community health care team, helping everyone have easy access to the patient’s care history.
  • A cardiac team where all health providers have expertise in cardiac care, including our community care partners at Saint Elizabeth.

The PPATH Program supports an integrated patient journey that begins in hospital and continues after discharge. The PPATH Program aims to:  

  • Provide better coordinated care between hospital and community care providers
  • Support patients to reach their health care goals, recover, and return to the activities they enjoy sooner
  • Reduce avoidable hospital readmissions
  • Reduce avoidable Emergency Department visits

By putting patients at the center of everything we do, we ensure that the patients and their families continue to receive the best possible care.
