The Chronic Kidney Disease Program at
Trillium Health Partners provides care for patients with diminished kidney
function and provides dialysis support when a patient's kidneys are no longer
functioning adequately. Our range of services includes hemodialysis, peritoneal
dialysis and dialysis at home, as well as care for patients who have had a
kidney transplant.
