The Heart Function Clinic provides outpatient follow-up care and education to patients with Congestive Heart Failure. The clinic has proven to be effective in increasing the quality of life for patients and decreasing their need to use acute care services.
Our services Include:
- Individual and group education to help you manage your chronic condition.
- Medication titration. “Titration” means your doctor will work with you to find the right dose of medication for you that manages your symptoms as much as possible, with as few side effects as possible.
- Individual consultations with a Nurse Practitioner or Registered Nurse and a heart doctor (Cardiologist).
- Ongoing diagnostic testing to assess and make referrals to tertiary centres for interventions.
- Optional individual counselling with dietary and pharmacy support services.
- Exercise programming
Who we Help
The clinic sees individuals diagnosed with heart failure and are followed by the Trillium Health Partners Hospital Cardiologists.
