At our Renal Care Centre satellite site, Trillium Health Partners provides a multi-disciplinary program of care, education, and support, in an outpatient setting, for patients who want to keep their independence and schedule their treatments around their lifestyle needs by having their dialysis at home.

Home dialysis therapies enable you to dialyze more frequently and for a longer period than hospital-based hemodialysis treatments. This can help improve your health outcomes.

Our program has dedicated staff that will ensure patients have sufficient training and support to be successful in the long-term.  We also have partnerships with community care agencies to provide dialysis support in your home if you need it.

The Renal Care Centre offers both a Home Peritoneal Dialysis Unit and Home Hemodialysis Unit, as well as a Transition Unit that helps prepare you for home hemodialysis training.

Each patient is assigned a primary nurse as they come onto the program.

Patients of the Home Peritoneal Dialysis Unit have regular clinic visits every four to six weeks with the renal health team.

Patients of the Home Hemodialysis Unit will be visited by a nurse at home once every three months. The unit offers both conventional hemodialysis that you do during the day-time, and hemodialysis that you do at night while you sleep.​

A Home Peritoneal Dialysis Unit Nurse is On-Call: evenings, weekends and holidays for telephone support.

Home Hemodialysis Unit
Monday to Saturday, 7:00 am – 6:00 pm

Transition Unit
Monday to Saturday, 7:00 am – 6:00 pm
