The
goal of the Multi-Care Kidney Care Clinic (formerly the Kidney Care Clinic) is
to help you be as healthy as possible and delay the worsening of kidney disease
if you are living with advanced kidney failure.
Our
team will help you and your family understand the treatment options available
to you, such as the different types of dialysis and transplant. We want to help
prepare you and your family for the time when you may need to begin dialysis.
Each patient of the clinic is assigned a primary nurse who helps
organize their care along with the support of the kidney specialists (nephrologists),
dietitians, pharmacists, and social worker.
