At our Regional Nephrology Clinic, we provide a single point of entry into
Trillium Health Partners’ Chronic Kidney Disease Program, which makes it easier
and faster for doctors and patients and their families to get the care the
need, when they need it.
At
the Regional Nephrology Clinic you can be sure you are getting the best
possible care. We follow chronic disease management best practices and the
recommendations from the Ontario Renal Network, a province-wide group that sets
consistent standards and guidelines to support the delivery of high quality
kidney care.
Each
year, our team of doctors and nurses monitors 3,000-4,000 patients.
If chronic kidney disease is
caught early, treatments and changes to your lifestyle, such as improving your
diet and staying active, may help slow its progress. Our goal is to help you
feel as healthy as possible for as long as possible.
Who we Help
Patients with early chronic kidney disease that
is mild to moderate (stages 1, 2 and 3).
