Senior’s Mental Health Outreach

Offered through our Queensway Health Centre site the Seniors’ Mental Health Outreach program is for seniors over the age of 65 experiencing poor mental health or mental illnesses who are at risk of requiring admission to hospital for psychiatric treatment.

The team consists of a specialized Geriatric Psychiatrist, a Nurse Practitioner, Registered Nurses, Social Workers and Occupational Therapists. The team provides consultation, education, and care coordination for clients, families, care givers and other health care providers.

We aim to enhance or maintain positive mental health for the seniors we serve, while respecting their culture, dignity, social justice, and equity.​

Who we Help

Seniors over the age of 65 with complex mental health, additional and behavioural issues.

Individuals under 65 who are diagnosed with an early onset mental illness may also be eligible for the program.
