ReLinC Services is a community-based case management service for people aged
16 and over, with a history of serious and persistent mental illness, often a
psychotic disorder.
Case Managers work in partnership with clients to develop the skills and
supports they need to live successfully in the community.
The focus is on goal based activity and community integration that
supports a client’s recovery and eventual discharge. They meet with clients either in their own
home or in the community.

residents (excluding Malton) and those in Etobicoke part of Local Health

Referrals
Clients are referred by health care
professionals, family and community agencies.
