Seniors and Rehabilitation Day Hospital

The Seniors and Rehabilitation Day Hospital is an outpatient program designed to help patients safely transition from hospital to home and community.

This program offers coordinated health services for people recovering from functional decline due to recent injury or illness. Services are designed to fit specific patient needs.

All patients admitted into the Seniors and Rehabilitation Day Hospital Program will be assessed by the Nurse (RN). Treatment must include 2 or more of the following team members: Occupational Therapy (OT), Physiotherapy (PT), Speech-Language Pathology (SLP), and Recreation Therapy.(RecT)

The goal of the program is to give patients and families the knowledge and skills to safely manage their health. The team aims to help patients successfully re-integrate into their community. The program offers an individual assessment and treatment plan to help patients in their recovery. Therapy may include exercise programs, practice of daily activities, and education specific to patient needs. Progress will be evaluated regularly by the team.

Who We Help

Patients may be eligible for the program if they have had a recent hospital stay or Emergency Department visit at Trillium Heath Partners.

To be eligible for the Seniors and Rehabilitation Day Hospital, patients must:

  • Be referred by their Care Team at Trillium Heath Partners
  • Be over 18 years old
  • Be medically stable
  • Have shown recent progress in recovery and have clear rehab goals
  • Require at least two of the provided therapies (PT, OT, SLP, RecT)
  • Be able to tolerate at least a 90 minute program plus transportation time
  • Commit to attend regular sessions and complete recommended home activities and/or exercises
  • Have a family physician

Services Offered Include:

Nursing

  • Education about your diagnosis and medications
  • Health management and monitoring
  • Assist with referrals to other health care providers

Occupational Therapy

  • Treatment of visual and thinking skills
  • Treatment of physical and daily living skills
  • Discussion about returning to work and driving, with referrals to appropriate resources

Physiotherapy

  • Treatment of physical difficulties and impairments
  • Education and home exercise program provided

Speech Language Pathology

  • Treatment of speech, language, and cognitive communication disorders
  • Treatment and management of swallowing difficulties

Therapeutic Recreation

  • Treatment of physical, thinking, social and/or emotional health
  • Education on meaningful activity using leisure
  • Assist with referrals to community programs

Resources for Patients and Their Families

Q: How can I attend the Seniors and Rehabilitation Day Hospital (SRDH)?
A: The SRDH must receive a referral signed by a physician. Referrals are prioritized, with priority given to patients who have been discharged from the Emergency Department or Acute Care Units. If the referral meets the criteria, a schedule is made and the team will contact the patient. The waiting time varies depending on the demands on the service.

Q: How often do patients come to the SRDH?
A: In general, patients attend for a period of 8 weeks. Shorter or longer periods of time may apply, depending on the status of the patient and their rehab goals. The frequency and length of time of the appointments depends on the number of therapies the patient is receiving.

Q: How do patients get to the SRDH?
A: Patients are responsible for their own transportation, and parking expenses. Some patients come by Transhelp.
